Introduction From 1st August 2004 working patterns and on-call rotas for all Anaesthetic SpRs at Papworth will be compliant with the European Working Time Directive. In order to achieve this, the three SpRs on regional training rotations who visit Papworth for three months, will participate in a full-shift with four clinical fellows (i.e. SpRs who are at Papworth for 6-12 months). All out-of-hours on-call will be based on the ICU. When not involved in an ICU rotation, clinical fellows will continue to provide on-call anaesthesia and transplantation services on a 24-hour on-call basis. The introduction of Donor Care Practitioners/Physiologists in August will reduce, and ultimately eliminate, the requirement for clinical fellows to join the donor organ procurement team on visits to other hospitals. Each period of work has a weekend off at the end of it. SpRs will work an average of 48 hours a week (Band 1A). Please note that banding is subject to change - please refer to the latest information.
Examples On the premise that four week full-shift blocks never run consecutively and never start after the 9th week of the 13 week attachment, there are 10 possible 13 week working patterns:
During the five weeks designated "X"; two (2) will be a "Long Theatre Week" and three (3) will be annual/study/bank holiday leave.
SpRs on Regional RotationsAn average 3 month attachment at Papworth comprises 13 weeks. During each 13 week block SpRs will be allocated the following periods of work:Two (2) blocks of four (4) consecutive weeks on a full-shift. Two (2) weeks in theatre with no on-call commitments. Upto three (3) weeks annual/study/sick leave. There will be 4/13 weekends of on-call duty (this was previously 5/13). A total of 8 days annual leave and 2 bank holidays are built into the attachment. Up to 2 consecutive weeks of leave may be accommodated. Approval for annual/study leave will be granted on a 'first come, first served' basis. A total of 5 days of study leave is incorporated into each 13 week attachment. The third week of annual/study leave will be allocated at the discretion of the Department. No leave may be taken during the four week full shift block. Four week full shift blocks may not be split. The first week of the 3 month attachment will be either "Long Theatre Week" or "Weekend Day Call". It is unlikely that SpRs will start a four week full shift block after the 9th week of their attachment. In fairness to colleagues, any unplanned leave taken during the following periods must be made up later in the attachment:Friday, Saturday or Sunday in week "Weekend Day Call" or "Weekend Night Call" Monday, Tuesday, Wednesday, or Thursday in week "Weekday Night Call" Average hours of work = 54 hours per week. Clinical Fellows / Senior SpRs At any one time four (4) clinical fellows will participate in the four week full shift working pattern. Clinical fellows will, in addition, provide daytime ICU cover from 0700-2000, Monday to Thursday. At other times, clinical fellows will have daytime duties in theatre and contribute to a 1:5 on-call rota. A compensatory day off will follow every 24 hour period of on-call duty. Average hours of duty - 70 hours per week.
